| This session will highlight several large, community-wide initiatives to improve the system of care for PHWHA in communities in the US. Speakers will discuss the process of designing and coordination initiatives, challenges on the ground, and initial national evaluation data results. Strategies to reach and retain hard-to-reach populations in care will be shared. Speakers will discuss the stage of engagement in care cascade graphic included in Dr. Edward Gardner's 2011 Clinical Infectious Diseases article and provide brief individual case studies to illustrate where and why we are losing people living with HIV/AIDS to care, and strategies to best address the problem. Speakers will be featured grantees of AIDS United Access to Care Initiative and the session will be moderated by Maura Riordan, Vice President of Access and Innovation. |
